ABSTRACT CONTEXT: Myiasis is caused by larval infestation that usually occurs in exposed wounds. Dermatobia hominis is the most common fly species responsible for this parasitic infection. Genital piercing is an ornamental practice used in certain social circles. At placement, it transverses the skin surface and, as such, may be related to complications. CASE REPORT: We report a case of a 31-year-old man with a history of wound infection secondary to genital piercing who was exposed to an environment with flies, leading to myiasis. Mechanical removal and systemic antiparasitic drugs are possible treatments for myiasis. However, prevention that includes wound cleaning and dressing is the best way to avoid this disease. CONCLUSIONS: Genital piercing can lead to potential complications and myiasis may occur when skin lesions are not properly treated.

RESUMEN

ABSTRACT Objectives The aim of this study is to report an unusual form of penile cancer presentation associated with myiasis infestation, treatment options and outcomes. Materials and Methods We studied 10 patients with suspected malignant neoplasm of the penis associated with genital myiasis infestation. Diagnostic assessment was conducted through clinical history, physical examination, penile biopsy, larvae identification and computerized tomography scan of the chest, abdomen and pelvis. Clinical and pathological staging was done according to 2002 TNM classification system. Radical inguinal lymphadenectomy was conducted according to the primary penile tumor pathology and clinical lymph nodes status. Results Patients age ranged from 41 to 77 years (mean=62.4). All patients presented squamous cell carcinoma of the penis in association with myiasis infestation caused by Psychoda albipennis. Tumor size ranged from 4cm to 12cm (mean=5.3). Circumcision was conducted in 1 (10%) patient, while penile partial penectomy was performed in 5 (50%). Total penectomy was conducted in 2 (20%) patients, while emasculation was the treatment option for 2 (20%). All patients underwent radical inguinal lymphadenectomy. Prophylactic lymphadenectomy was performed on 3 (30%) patients, therapeutic on 5 (50%), and palliative lymphadenectomy on 2 (20%) patients. Time elapsed from primary tumor treatment to radical inguinal lymphadenectomy was 2 to 6 weeks. The mean follow-up was 34.3 months. Conclusion The occurrence of myiasis in the genitalia is more common in patients with precarious hygienic practices and low socio-economic level. The treatment option varied according to the primary tumor presentation and clinical lymph node status.

RESUMEN

Abstract: Myiasis refers to Diptera larvae infesting vertebrate animals. There are two forms of the disease: primary and secondary. In primary myiasis, fly larvae invade and develop in healthy tissue; in secondary myiasis, flies lay their eggs in skin ulcerations, and the larvae develop in tissue necrosis products. Furuncular myiasis is a type of primary myiasis. Treatment for it consists of techniques such as the production of localized hypoxia to force the emergence of the larvae, and mechanical or surgical removal of the maggots. These techniques, however, are painful and often unsuccessful. We propose a new technique for extraction of myiasis larvae, which might facilitate the surgical procedure and constitute a virtually painless and aesthetic option for the patient.

RESUMEN

Las miasis son infestaciones en vertebrados vivos causados por las larvas de diversas especies de moscas (dípteros). El objetivo del trabajo es presentar la evolución de este caso por ser una enfermedad muy rara en Cuba. Se presenta una paciente femenina, piel blanca, de 70 años de edad y antecedentes de hipertensión arterial crónica. Hace nueve meses le fue diagnosticado un proceso neoformativo de vulva, motivo por el cual fue intervenida quirúrgicamente hace 39 días en otro centro hospitalario. Ahora acude al cuerpo de guardia por constatarse aumento de volumen en la zona quirúrgica, prurito intenso y secreciones fétidas. Por este motivo fue ingresada para estudio y tratamiento en sala. Se le diagnosticó miasis vulvovaginal. Tuvo una evolución satisfactoria en sala gracias a las curas locales y el tratamiento con antibióticos(AU)


The myiasis are infestations in living vertebrates caused by the larvae of several species of flies (Diptera). The aim of this paper is to present the evolution of this case as a very rare disease in Cuba. A case is presented of a female patient, white skin, 70 years old having a history of chronic hypertension. Nine months earlier, she had been diagnosed with vulvar neoformative process, for which she underwent surgery -39 days before this consultation in another hospital. She went to the emergency room due to an increased volume in the surgical area, severe itching, and fetid secretions. She was admitted in hospital for study and treatment. She was diagnosed vulvovaginal miasis. She had a satisfactory evolution thanks to local cures and treatment with antibiotics(AU9

RESUMEN

Intestinal myiasis is usually an accidental phenomenon which is transient and asymptomatic. It occurs due to intake of contaminated food or water containing fly larvae or eggs. We report a case of invasive intestinal myiasis presenting with complaint of pain, blood-mixed mucous discharge per rectum along with passage of maggots in stool. On proctoscopy an irregular fungating rectal growth with maggots was detected in rectum and anal canal. Excision of growth with diversion colostomy was done. On histopathology numerous variable-sized larvae in different stages of development were detected throughout the inflamed and necrosed bowel wall. No tumor was seen. With maintenance of proper hygiene, vermicide anti inflammatory and purgatives were given. Patient became alright after colostomy closure. The symptomatic presentation as a mass lesion and necessity for surgical intervention prompted us to report this rare case.

O relato mostra um caso de oftalmomiíase pós-traumática em um paciente etilista crônico, com fratura orbitozigomática, descrevendo a importância do tratamento precoce e a necessidade de associar Ivermectina à remoção cirúrgica das larvas.


The report shows a case of post-traumatic ophthalmomyiasis in a chronic alcoholic patient with orbitozygomatic fracture, it describes the importance of early treatment and the association of Ivermectin and surgical removal of larvae.

RESUMEN

The genital localized myiasis is a rare zooparasitic dermatosis. This case reported to a penile myiasis by Dermatobia hominis larvae in a 62 year-old man, who is a retired military official from São Gonçalo city, Rio de Janeiro state, Brazil. This patient was guided to the sexually transmitted diseases (STD) clinic from Universidade Federal Fluminense (UFF) by professionals from a urology outpatient clinic of a public hospital from Rio de Janeiro state. The exam showed a 2 cm diameter nodular lesion with a hyperemic area in the glands penis that had arisen 20 days before the exam. It evolved in a favorable way after larvae removal, through previous local anesthesia and slight pressure in lesion's base with the extraction of the larvae. Penile myiasis is not a very frequent situation, although it must be considered as an option in differential diagnosis of genital infectious diseases, even among people of low economical level, as in the case presented.

RESUMEN

A case of anterior internal ophthalmomyiasis is described. A 27-year-old female from Northern Brazil presenting with anterior uveitis and vitritis had a fly larva surgically removed from the anterior chamber of the left eye. The species was Cochliomyia hominivorax. The larva was covered by macrophages and foreign body giant cells characterizing a foreign body granulomatous reaction.


Um caso de oftalmomiíase interna anterior é descrito. Uma mulher de 27 anos proveniente da região Norte do Brasil com uveíte anterior e vitreíte teve uma larva de mosca cirurgicamente removida da câmara anterior do olho esquerdo. A espécie identificada foi Cochliomyia hominivorax. A larva estava coberta por macrófagos e células gigantes de corpo estranho, caracterizando uma reação granulomatosa de corpo estranho.

OBJETIVO: Miíase é a invasão de tecido humano pela larva de uma mosca da ordem Díptera. O envolvimento ocular não é comum. Lesões do aparelho lacrimal geralmente estão associadas a traumas e raramente são devidas a infestações parasitárias. O objetivo deste trabalho é relatar um caso de laceração canalicular causado por larva de Dermatobia hominis. DESCRIÇÃO: Criança do sexo feminino, 8 anos de idade, apresentou-se com celulite pré-septal não responsiva a antibioticoterapia. Foi observada uma larva de D. hominis no saco lacrimal. Realizou-se a extração cirúrgica da larva e observou-se lesão do sistema de drenagem lacrimal. COMENTARIOS: A infestação parasitária em via lacrimal é rara. A extração cirúrgica da larva é recomendada como tratamento de escolha. Apesar de incomum, a oftalmomiíase deve ser considerada como um diagnóstico diferencial possível em celulites não responsivas ao tratamento convencional, especialmente em áreas endêmicas. Esta é a primeira descrição de lesão do sistema de drenagem lacrimal por larva de D. hominis.

Myiasis is an infestation of tissues with larval stage of dipterous flies. This condition most often affects the skin and may also occur in certain body cavities. It is mainly seen in the tropics, though it may also be rarely encountered in non-tropical regions. Herein, we present a case of cutaneous furuncular myiasis in an Iranian male who had travelled to Africa and his condition was finally diagnosed with observation of spiracles of larvae in the lesions

To study cases of furuncular myiasis by tumbu fly [Cordylobia anthropophaga] among Pakistani troops in Sierra Leone, Africa. Design: Observational study. Place and Duration of Study: Pakistani troops in Sierra Leone, Africa. The duration of the study was 1 year, from July 2001 to August 2002. Subjects and Two hundred and forty-eight cases of furuncular myiasis were observed among Pakistani soldiers. All the patients were male. The lesions were treated by removal of the larva followed by twice daily application of an antibacterial ointment. The extraction was done by injecting 2% xylocaine over the base of the nodule. Care was exercised in not damaging the larva while extracting. The ages were from 22 to 43 years. Mean age was 29.42 years. The patients presented with a solitary lesion [178 out of total 248 cases, 71.8%] or multiple lesions [70 out of 248 cases, 28.2%]. The lesions were almost always on the non-exposed parts of the body [239 cases out of total 248 cases, 96.37%]. One case presented with a lesion on the face and 8 cases had lesions over dorsum of hands or feet. This disease is common in tropical Africa. The knowledge of the disease, its management and prevention is necessary for all the individuals who are planning to visit tropical Africa

Rara em humanos, quando identificada é prevalente em regiões tropicais, na zona rural, tendo como fator predisponente mais freqüente a rinite atrófica. Myiasis is most prevalent in Mexico, central and south America, tropical Africa, and the southwestern United States. Although dermal myiasis is rare in most of the United States, it is a disorder that may be seen in international travelers. In the United States, external myiasis is usually caused by the cattle botfly. We report here a case of ophthalmomyiasis involving the left upper eyelid of a child. We examined a six-year-old boy who presented to the Massachusetts Eye and Ear Infirmary (MEEI) in September 1998. He complained of persistent swelling of his left upper eyelid for the previous ten days. The edema and erythema were unresponsive to warm compresses and oral antibiotics. Ocular examination revealed a mild preseptal cellulitis of the left upper eyelid with a small draining fistula. On slit-lamp examination, we found one larva protruding intermittently from the fistula site. The larva was extracted with forceps, wrapped in a moist towel and sent in a jar to the parasitology laboratory. The specimen was identified as a Cuterebra larva by a parasitologist at the Harvard School of Public Health. One week later, the patient's eyelid edema and erythema had completely resolved.

RESUMEN

Os autores apresentam um caso de oftalmomiíase com fístula orbito-maxilo-bucal por larvas da espécie Dermatobia hominis. O tratamento clínico foi realizado com éter sulfúrico, para retirada das larvas, curativos com óxido amarelo de mercúrio 1 por cento e medicação sistêmica que incluía antibiótico e antiinflamatório via oral. Apesar da resolução satisfatória com involução espontânea da fístula, foi necessária enucleação do globo ocular.
